All participants will need to sign a 'Participation Agreement' prior to engaging in hockey related activity.

Please click on the 'Participation Agreement' to do this.  This will be required before any players can commence their first session with SJB Academy.  This only needs to be done once, so players that have completed this do not need to do it again.  All players must register their intent to train.  Players who have not registered prior to the session will NOT be permitted to play.  (Note:  On the form please ignore Question 5, and select SJB Academy HC in Question 6)
